BAPTISM INFORMATION FORM

DIRECTIONS: Please provide the following information to complete your baptism form. Note that all requests for baptisms must be approved by Pastor Jefferson prior to submission of this form. All requests need to be submitted 7 days in advance of the requested date, but we highly encourage early submittals. Upon submission, your form will be reviewed for final coordination.

The wording for the certificate can be found on pages 683 and 684 of the 2008 AME Discipline. Your answers to the following questions will determine how the certificate is worded.

• Only Questions 1 - 3 are required for adults

• Questions 1 - 7 are for parents or guardians of children from 12 to 18 years old

• All questions are for the parents or guardians of children from infants to 12 years old
